Position Description:

We have a unique career opportunity for an experienced hands-on development professional to establish a development program to support our mission and services.

The Director of Development will be responsible for all aspects of fundraising for this organization with a $20 million operating budget. This individual will build a development function focusing on foundation and government grants, corporate support and individual philanthropy.  The Director of Development will work with the Executive Director and senior staff in developing and implementing a fundraising strategy.

Responsibilities include:

·         Develop an overall fundraising strategy, including goal setting and performance measurement

·         Implement a fundraising plan including

o        Government grants

o        Foundation grants

o        Corporate support

o        Individual giving

o        Major gifts

o        Planned giving

o        Building ground work for future capital campaign

·         Develop, write, coordinate and submit all grant proposals and appeals

·         Research new funding prospects in all areas (foundation, corporate, government and individuals) and for all projects

·         Track proposals, grants and develop reports to funders

·         Maintain and expand donor database, gift acknowledgements and donor recognition

·         Develop and manage all aspects special events and donor cultivation events.

·         Over see the creation, preparation, and dissemination of development materials, including participation in the development of overall organizational branding and internet strategy

·         Development of multi-state fundraising program based on Pathway’s locations and areas where our Housing First philosophy has been implemented.

·         Provide support and direction to Board on philanthropic matters.

Qualifications:

Minimum of seven years progressive responsibility and demonstrated success in fundraising required. The candidate should have superior writing, organization and communication skills. He/she will have the ability to effectively communicate the mission, goals, strengths and potential of the organization and have demonstrated knowledge of fundraising, based on a combination of educational training and practical experience. Fundraising experience in a social services organization is desirable.  Strong computer skills required. Facility with word, excel and fund raising database software is preferred.

TITLE:  PSYCHIATRIST

REPORTS TO:  DIRECTOR OF HEALTH SERVICES

 

 

Pathways To Housing Inc., a recognized national leader in the housing first, harm reduction service delivery model for individuals with mental illness and substance abuse problems, announces the opening of the Philadelphia Pathways to Housing Program.

 

We are seeking a Psychiatrist to work to as an integral part of the trans-disciplinary ACT team, providing psychiatric care and support for consumers and to promote wellness, recovery and rehabilitation.  Responsibilities are implemented within the framework of agency policy, professional ethics and regulatory guidelines. The psychiatrist acts as the clinical co-leader of the ACT team in conjunction with the Team Leader.

 

 

The Psychiatrist is an integral part of the trans-disciplinary ACT team, providing psychiatric care to clients in order to promote wellness, recovery and rehabilitation.  Responsibilities are implemented within the framework of agency policy, professional ethics and regulatory guidelines.  The Psychiatrist may be assigned to more than one ACT team and collaborates with the Team Leader to ensure optimal use of psychiatric time.

 

Responsibilities:

    Complete an Initial Psychiatric Evaluation and Mental Status Exam for new clients and meet with clients at least monthly.

     Provide treatment for assigned clients as prescribed by the treatment plan and document the client chart with an appropriate note for each encounter.

     Write orders and prescriptions for medications, and monitor for efficiency and side effects.

     Work closely with the team and the client in establishing and following an optimal treatment/service plan.

      Participate in check-in meetings and case-reviews, provides consultations and make home visits with team members as needed.

      Collaborate with the nursing staff to coordinate prescriptions and orders, and to oversee the overall health of the clients.

      Establishes a work schedule, in collaboration with the Team Leader that ensures monthly psychiatric visits for all clients.

     Engage identified clients in psychiatric services, both onsite and in the community.

     Participate in facilitating appropriate hospitalizations, crisis interventions and referrals.

     Communicate with hospital staff to ensure optimal treatment and discharge planning.

     Establish/Lead education and support groups on health related topics in the Health & Fitness Initiative, e.g. smoking cessation, medication education

     Provide in-services and trainings for staff.

      Perform other related work as assigns.

 

 

Qualifications:

 

     Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.

     Board Eligible/Board Certified in Psychiatry.

     Experience or training in community psychiatry preferred.

     Experience with dually diagnosed population preferred.

     Excellent written and verbal communication skills.

Title:  Registered Nurse/Service Coordinator (RN/SC)

Reports to:  Director of Health Services & Team Leader

 

The Registered Nurse/Service Coordinator is an integral part of the trans-disciplinary Assertive Community Treatment (ACT) team, who provides flexible, community based ACT services designed to promote stability, recovery and community integration of formerly homeless individuals who have a mental illness and abuse substances. The RN/SC maintains a primary caseload of 4-8 clients and in collaboration with the transdiciplinary team coordinates and provides, in a compassionate and responsible manner, ACT services for all clients on the team. Responsibilities are implemented within the framework of agency policy, professional ethics, legal scope of practice and regulatory guidelines. Examples of work are listed, but not limited, to those below:

 

 

RESPONSIBILITIES:

 

·         Collaborate with ACT team members to coordinate client health services.

·         Supervise/Evaluate LPN staff collaboratively with Director of Health services

·         Perform Health Assessments.

·         Maintain, in collaboration with LPN, schedule for client prescriptions

·         Transcribe Provider order and update Medication Administration Records

·         Administer IM’s and vaccines, plant and read PPD’s, perform phlebotomy as per Provider orders.

·         Monitor client vital signs, glucose, side effects of medications, etc and report findings as required

·         Provide continuous client education re: mental illness, medication, and health issues

·         Work with team and client in developing/following the optimal treatment/service plan

·         Collaborate with psychiatrists and nurses in developing/delivering Health & Fitness Initiatives

·         Engage identified clients in medical services, on site and in the community

·         Provide health related in-service training and education for staff

·         Serve as a health care resource to clients, families, staff and significant others

Service Coordinators needed to work with dedicated trans-disciplinary, service-oriented staff.  Positions are available for credentialed professionals – Masters and CASAC preferred.  Successful candidates will provide community based mental health services for formerly homeless population with psychiatric disabilities and substance abuse issues.  Knowledge of population and its issues essential. Ability to work independently and as a team player with diverse populations necessary.  Positions available in Brooklyn and Manhattan locations.

Service Coordinator, Vocational Specialist:

Service Coordinators are members of the Assertive Community Treatment (ACT) team who provide flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and abuse substances .The Service Coordinator (SC) maintains a primary caseload of 10-12 clients and in collaboration with the transdisciplinary team coordinates and provides, in a compassionate and responsible manner, ACT services for all clients on the team.    In addition to performing routine team duties, the SC delegated as Vocational Specialist has lead responsibility for integrating vocational goals and services with the tasks of all team members. This staff member provides needed assistance through all phases of the vocational service. The Director of Vocational Services and Human Resources provides direction, supervision and oversight of the vocational service. Examples of services and responsibilities are listed below.

Service Coordinators are members of the ACT team and work together to provide services in a flexible, compassionate, and responsible manner.  The Service Coordinator provides and coordinates all services and has a primary caseload of ten clients and shares responsibility with team for all other clients who have histories of homelessness, psychiatric disability, substance abuse, criminal justice, and other trauma.

Team Leader:

The Team Leader is responsible for the daily operation and clinical supervision of a transdisciplinary Assertive Community Treatment (ACT) team and the development and oversight of a program sufficiently flexible and dynamic to meet the needs of those individuals who have psychiatric disabilities and substance abuse issues who have not been well served or have been unable to access traditional mental health programs. The Team Leader provides supervision for the Service Coordinators and Program Assistant and is responsible for assuring that the agency values and philosophy of consumer choice; harm reduction and respectful concern are expressed in all aspects of team practice. TheTeam Leader maintains oversight to ensure that client self identified goals for recovery; rehabilitation and community integration are supported by appropriate services and best practices.

The Licensed Practical Nurse is an integral part of the trans-disciplinary Assertive Community Treatment (ACT) team, who provides flexible, community based ACT services designed to promote stability, recovery and community integration of formerly homeless individuals who have a mental illness and abuse substances.  The LPN assists with all medication, including all packaging, administering, and monitoring and assists team NP/RN in providing clients with medical services.  Responsibilities are implemented within the framework of agency policy, professional ethics, legal scope of practice and regulatory guidelines.
